Senior Pharmacist -Oncology
A fantastic opportunity to join a leading modern Hospital in Dublin in the provision of a pharmacy / aseptic service to chemotherapy patients.
Responsibilities and Deliverables
- Provide clinical oncology + aseptic-based services,
- Provide advice across MDT’s, e.g. Consultants, Doctors, Nurses.
- Ensure knowledge of up to date developments in best practice through continuing education and research activities as facilitated by the post.
Information Technology
- Be knowledgeable in the use of the hospital-wide E-Prescribing (ward), to include providing training and support to Clinicians in best practise use.
Clinical Service Delivery
Clinical Ward based:
- Participate in ward + clinical oncology pharmacy services.
- Undertake Medicines reconciliation of patients admitted to the hospital.
- Monitor in-patient prescriptions for their clinical accuracy and for cost-effective prescribing.
- Assesses patient’s response to therapy, interpret blood results, either adjusting drug doses or advising Clinicians as appropriate.
- Advise on therapeutic drug monitoring.
- Monitor and report any Adverse Drug Reactions experienced by patients.
Aseptic Services:
- Technical check + final release for individually dispensed aseptic products to include adult intravenous additives + cytotoxic chemotherapy.
- Clinically checks + validates prescriptions received from wards
- Manage queries on preparation details/stabilities/calculations etc ..
- Clinically screen chemotherapy prescriptions.
- Facilitate the continuous development of new oncology regimens + related policy.
Quality
- Participate in JCI Accreditation.
- Ensure the chemotherapy prescribing process is optimised through regular clinical and technical audits.
- Contribute to hospital – wide quality medication use + safety reports.
- Participate in the development of protocols + guidelines to deliver optimal + economical patient care, in regard to chemocare E-Prescribing.
Requirements
- Registration with the Pharmaceutical Society of Ireland.
- A minimum of 3 years relevant post-graduation hospital experience.
- Hospital Oncology Pharmacy experience is essential.
- Experience in implementing using e-prescribing is preferred.
- A relevant Diploma/ MSc is preferred
